From the View Client With Insurance Screen, click on the Add Action Button to start the Add Policy Wizard. This screen is Step 3.

This step contains more information which applies to the coverage risk.  The information on this screen is optional unless it contains an asterik (*) next to the field.

Fill in the fields or use the drop down lists and then click on Next.

Click on the drop down list to choose from the tobacco types.

Qualified Plan Type

Click on the drop down list to choose from the type of qualified plan, if any.

Approval Basis

Click on the drop down list to choose from the Risk Class.

Participants

Enter the number of participants in this coverage if it pertains.
